    INSTRUCTIONS.

    Choose one of these two orderform.
    1 - Click on the picture to download it..
    2 - Save it to your computer.
         Then you have two choices :

  • Open it and fill it out on the screen.Each time you make a checkmark, the total is displayed with and without taxes at the bottom of the order form..
    When you have finished, click the GREEN button at the bottom of the form to send it by email.
    * The yellow button at the bottom of the form is used to clear all fields if you wish to start over.

  • Print it if you prefer to fill it out by hand. Once finished, send us a photo of the form, or save the document and email it to the following address: formweb@labelmystuff.com
